Clinical Nurse Care Manager

We’re seeking a compassionate RN to support seniors with personalized care. This flexible, part-time role (15–20 hrs/week) offers meaningful connections and impactful work—beyond the hospital setting.

MMajor Responsibilities:

  • Patient Assessment & Care Planning: Assess patient needs and develop comprehensive care plans tailored to individual requirements
  • Ongoing Assessments: Performing regular physical and psychological assessments to monitor client progress
  • Clinical Support: Providing medication management, in-home assessments, and client Led to nursing interventions
  • Team Coordination: Coordinate with multidisciplinary teams to ensure seamless patient care and effective communication
  • Progress Monitoring: Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes
  • Healthcare Navigation: Guiding clients through the healthcare maze and advocating for their needs
  • Medical Advocacy & Liaison: Serving as the crucial link between patients, families, and healthcare providers to ensure clear communication and understanding
  • Patient & Family Education: Educate patients and their families about health conditions, treatment options, and available resources; empowering clients and their families with knowledge to participate in their care actively
  • Fall Prevention: Implementing comprehensive fall care strategies and safety assessments
  • Care Coordination: Making referrals and recommendations based on individual client needs
  • Compliance & Documentation: Ensure compliance with HIPAA regulations and maintain confidentiality of patient information; maintaining accurate electronic medical records and all appropriate documentation
  • Utilization Management: Conduct utilization management reviews to optimize resource use while maintaining quality care standards
  • Team Collaboration: Participating in ongoing care conferences with primary care teams
